Schools and district officers caused the issue, because they missed the deadline, to submit attendance registers.
The department is reviewing the initiative’s internal governance, to ensure that the programme, is adequately resourced, and properly supervised.

The Minister says an investigation is underway,determine who was responsible for the issue.
